The Documentation Templates and Rules (DTR) Questionnaire APIs enable the ability to tell a provider/EHR exactly what clinical documentation is required in the computable form during prior authorization or coverage workflows.
Questionnaire operations are mechanism to express documentation requirements computably in a way that allows clinicians and other EHR users to navigate and quickly specify the needed information in a context-specific way. Create Forms/Questionnaire Parameters

parameter | One or more named inputs. Supply coverage and order references for discovery, or supply a questionnaire canonical URL when it is already known (required). |
name | Parameter name, select one of coverage, order, or questionnaire (required). |
valueCanonical | Canonical URL of a specific Questionnaire to retrieve. Used when the caller already knows which questionnaire is required. |
valueReference | FHIR reference for the coverage or order resource used to determine the required questionnaire. |
resourceType | FHIR resource type. This value must be Parameters. |
x-mock-scenario | Used in sandbox environments to return predefined mock responses instead of real backend processing. |
x-optum-trace-id | Unique identifier used to trace and correlate requests across Optum systems for troubleshooting and support. |
